From North Memorial Parkway:

1. Traveling south on the Parkway, take the Drake Avenue exit.
2. Turn right onto Drake Avenue.
3. At the second stoplight (Ivy Street), turn right.
4. At the four-way stop, turn left.
5. Continue past Milton Frank Stadium and turn left onto Westmill Drive.
6. Hospice Family Care is located at the back of the cul-de-sac.

From South Memorial Parkway:

1. Traveling north on the Parkway, take the Drake Avenue exit.
2. Turn left onto Drake Avenue.
3. At the second stoplight (Ivy Street), turn right.
4. At the four-way stop, turn left.
5. Continue past Milton Frank Stadium and turn left onto Westmill Drive.
6. Hospice Family Care is located at the back of the cul-de-sac.

From I-565

1. Traveling East on I-565, take the Jordan Lane Exit.
2. Turn right on Jordan Lane.
3. Turn left on Bob Wallace.
4. Turn right on Triana Boulevard.
5. Turn left on Ivy Street.  (This is near Domino's Pizza, before you get to Drake Avenue.)
6. Turn right on Westmill Drive before you get to Milton Frank Stadium.
7. Hospice Family Care is located at the back of the cul-de-sac.
